Jacobs struggled to fit his taped fingers into a glove on the sideline.
After one drive, he returned to the sideline.
Jacobs, who left after 19 carries for 73 yards and the game's only touchdown, leads the league in rushing with over 1,300 yards.
Rookie Zamir White replaced him on the next drive.
Jacobs has a calf injury and a hand injury.
Second-leading rusher Derek Carr had 17 carries for 77 yards.
